Frimley Health community services are looking to recruit a Dynamic, motivated nurse with advanced clinical and communication skills. The role of the Community Matron is to work as an autonomous nurse at an advanced level, making complex decisions and using expert clinical judgements to improve clinical and quality of life outcomes for individuals with complex health needs The Yateley Community Nursing team is employed by Frimley Health and is part of the Yateley integrated care team. Integration between both Community and Acute providers is key and the successful candidate will work on developing and leading these relationships. Within Yateley there is one primary care network serving a population of approx. 28,000. The locality has benefited from significant investment locally as a result of being a 'vanguard' site, this has led to development in alternative roles and an urgent care centre which meets the needs of the local population. The locality is supported by infrastructure of forward thinking GP's and clinicians who are looking to develop fully integrated services locally. The successful candidate must be able to demonstrate extensive post registration education within the sphere of long term conditions such as COPD, Diabetes and Heart failure amongst others. They will need to have strong leadership skills both to initiate and support change, and work collaboratively within the multi- disciplinary team within the Frimley Health system and key stakeholders. The successful candidate will have to make high-level clinical decisions, preventing avoidable hospital admission, as well as planned, managed discharges from the acute setting. The Community Matron will also support patients to remain independent and within their own homes/environment in the community. You will have responsibility for the day to day management of the community care teams to ensure that all patients within your locality receive high quality timely care.
