Job Description

Working within the Primary Health Care team and under the direct supervision of the District Nurse you will assess and deliver an agreed plan of care to individuals within a community setting. It is essential you are a Registered Nurse with community nursing experience being desirable. The opportunity to further develop and enhance clinical and managerial skills, and provide support for the District Nurse, within the community environment is available. A current Driving Licence is required.
