The nursing team has 1 Band 8a, 2 Band 7, 5 Band 6 nurses making up the senior nurse team. They are supported by 6 and 5 nurses and 5 HCA with advance nursing skills. This gives good peer support and development to the team of nurses. The integrated team across social services, voluntary sector and health input make up the team to plan the patient focused care plan. The extended primary care team consist of 2 Physiotherapist, 2 Occupational Therapists, 3 Rehab Support Worker Care Navigator and Admin. The Nurses component has the emphasis on experience senior nurses working with families to promote independent living. As well as bringing your clinical expertise, you will be a champion of integrated community care. You will be a strong team player, an excellent communicator who is adaptable, committed, well organised and work well under pressure. You will support the development, implementation and maintenance of high quality, safe and sustainable clinical services and provide clinical and operational leadership under the direction of the team leaders, clinical lead and general manager.
