Over the past two years, the Trust has been growing the frailty service to make significant improvements in the way that care is delivered. Successes have been seen in terms of length of stay reduction, improved education and awareness of frailty and improved environment for our frail patients We would like to recruit a clinical expert and highly motivated Pharmacist and independent prescriber to join our team of Frailty Practitioners (nurses and therapists) in the delivery of the ICHT Frailty service. This is a new expansion of the existing frailty clinical teams which will see the teams move forward into the Emergency Department. This will facilitate early identification and targeted intervention for frail older adults presenting to our trust Emergency departments. The role involves working as an expert practitioner and clinical leader in the Frailty service, delivering person centred care, frailty screening and comprehensive assessment for older people and people with frailty across the Frailty Pathway at ICHT. Our frailty service is developing rapidly, therefore the post holder will actively contribute to service development and transformation work. The candidate will not only have an interest in improving care in frailty, but will also be an enthusiastic educator, as development of the local workforce to upskill in care of older adults will be a vital aspect of the role. You will have significant experience in working within an Emergency or Acute environment. You will have experience with older adults who are acutely unwell and will have undertaken the appropriate post registration education and practical experience to evidence your skills. The post is based within Urgent and Emergency Care, alongside the Department of Elderly medicine, with a strong interdisciplinary working and support networks. The role will require cross site working between St Mary's Hospital and Charging Cross Hospital.
