About you: The post holder is responsible for contributing to the development, implementation and evaluation of evidenced based programmes of care, working in collaboration with the Team Leaders and Community Matrons as a member of the Team. The post holder is designated to take charge in the absence of the Community Nursing Sister who has continuing responsibility.
