Job Description
An exciting opportunity has arisen for the position of Transfer Care Navigator within the Surgical Directorate. Successful candidate will be based on George Perkins Ward at St. Thomas's Hospital and will be required to support ward staff and facilitate discharge planning. George Perkins Ward is a 28 bedded Trauma Orthopaedic Ward which is one of London's busiest Trauma admission wards. This is a fast paced clinical environment for patients with orthopaedic emergency trauma cases. Our service provides orthopaedic emergency care to patients with complex nursing and discharge needs. The Ward is focused on multidisciplinary working as staff work very closely with Physiotherapist, occupational therapist and Specialist Older Persons Teams.
