Job Description

We are looking for an enthusiastic and motivated Registered nurse to join our supportive and dynamic multidisciplinary team. We are an integrated health and social care team providing clinical assessment and triage for patients face to face to prevent unnecessary hospital admissions or to facilitate appropriate discharges.We also work with patients reaching the end stages of their life. We complete the fast track assessments and formulate a discharge plan. Are you a Band 6 or experienced Band 5 Registered nurse looking for a new career challenge? We will consider secondments /rotations in order to encourage improved integration between acute and community services. Hours: 5 x 7.5 hours shifts between the hours of 8 - 6pm over 7 days. The successful candidate will have the ability to work as a confident autonomous practitioner. You will need high level clinical assessment and problem solving skills to manage complex patients who require assessment of need prior to discharge. You will work both within the Single Point of Access, clinically triaging referrals as well as supporting patients in the community who are at risk of hospital admission following a crisis, or need support home from hospital. As a nurse, this post allows you to draw on skills gained on skills gained from both acute and community experience, due to the broad range of patient presentations and work both over the phone and face to face in the community and acute hospitals. You will also work with Specialist Nurses and Physiotherapists, enabling you to expand your knowledge in multidisciplinary complex care. You will need to demonstrate flexibility, excellent time management skills and excellent communication and interpersonal skills. This is an exciting and fast paced team, where each day can present new challenges and learning opportunities. You will be a part of the evolving Single Point of Access service; there are great opportunities for service development experience. This post would be a fantastic opportunity for an experienced Band 5 who has completed core rotations and who is looking to progress to the next step. It would also be an excellent post for a Band 6 looking for a fresh challenge with new opportunities.
