Job Description
An exciting new opportunity has arisen for an innovative and dynamic Band 4 Support Practitioner within the Tissue Viability Team. This is a new role developed to support with a proactive Tissue Viability team of specialist nurses, who provide specialist Tissue Viability care within community services in Barnet. The team pride themselves on the provision of innovative services that best meets the needs of our developing population. This post holder will be working with the community Tissue Viability team who are based at East Barnet Health Centre. The role will involve rotation between our new wound management clinic, leg ulcer clinics and working in partnership with clinicians in community home visits, supporting district nursing, nursing homes, community in-patient units and the hospice. This autonomous position requires a highly motivated, dynamic practitioner who can demonstrate effective time-management, strong clinical skills, with the ability to communicate well across multi-professional boundaries. You will also be will involved in educating and empowering both healthcare professionals and patient's, and you will be involved in teaching informally.
