Job Description

An exciting opportunity has arisen for experienced Patient Flow Co-ordinators to join our team. The post holders are expected to provide efficient and effective flow coordination, working alongside our Medical, Nursing and Site Management teams to ensure that effective discharge planning and communication are maintained in order to provide a smooth, coordinated journey for the patient from admission to discharge or transfer to a base-ward. The post holders will be proficient in dealing with confidentiality issues and able to provide excellent customer care service, ensuring patient's details are accurately recorded and the relevant documentation is created to support safe patient care. The Acute Medical Unit is an exciting and stimulating place to work and benefits from excellent team working. This role would suit someone who has excellent interpersonal skills, is proactive and looking for a challenge. The successful candidate will demonstrate excellent communication skills, the ability to remain calm under pressure and experience of dealing with the general public and with difficult/distressed people. A flexible approach to work and experience of working in a busy environment and with minimal supervision are also essential. The post holder will be required to undertake additional hours to cover for colleagues during periods of annual/sick leave. Hours of work: Evenings (1630-2130) and weekends (1030-1930) (14hrs per week average - 3 week rolling rota - see job description for rota) To include Bank Holidays.
