Job Description

An opportunity has become available in the Lancing Community Team for a District Nurse/Community Sister who is looking to utilise their clinical skills to the full. The applicant will be able to demonstrate and communicate a depth of knowledge and experience of case managing patients and managing a caseload with a variety of long term conditions. The successful applicant will be dedicated in their approach to the role and be able to utilise their specialist skills as a district nurse/community sister This is a chance to join a innovative, forward thinking and proactive Community Multi-disciplinary team, helping shape a better future for patients' with chronic health conditions. The post is a Full time post and covers the GP practices in the Lancing Primary Care Network. You will not only be involved in caring for patients at home but be part of a team determined to see a coordinated approach to this group of complex patients and avoiding unnecessary admissions. You will be working in an environment where patients' needs can be efficiently met to help them and their carers manage their progressive disease effectively. The applicant will work closely with our colleagues in Primary care and West Sussex County Council and Sussex partnership NHS Trust. It is important for the applicant to be able to forge good relationships with key care partners to ensure continuity in care of patients on the caseload.
