Service/Ward/Department Summary: An exciting opportunity has arisen for a Band 6 Occupational Therapist to join the community Occupational Therapy Team on a fixed term contract until end of March 2021 . The Community Occupational Therapy team is based at the Link Centre and part of the Integrated Therapy Hub, which comprises of Community Neuro Rehab, Falls, Stroke, Dietetics, Speech and Language Therapy, Macmillan AHP's, Community Physio. You will directly report to the team leader within the community occupational therapy team and will work in line with the agreed objectives of the OT Service/Oldham Care Organisation. You may be required to work in other community settings including the patient's home as required by the services. Role Summary: An exciting opportunity has become available to recruit a Band 6 Occupational Therapist to work as part of an established team providing assessment and treatment to patients in a community setting and being responsible for your own clinical caseload. You will work as a member of the Community Occupational Therapy Team, promoting independent living and well being through individualised assessments which may lead to the provision of specialist equipment or environmental adaptations to the home, Working with both adults and children experiencing difficulties managing everyday activities due to disability/illness within their home environment this is a varied and diverse role. The post holder will actively liaise with other services in the community, primary and secondary care to ensure the necessary support is available. The post holder will provide support/supervision/training to peers and junior Occupational Therapy staff as required. Further details can be found within the job description and person specification There may be a requirement to work flexibly which may include evenings and weekends. Key Responsibilities of the Post: * Assess the functional abilities of people whose needs can be complex, changing and vary across the caseload. Assess the functional abilities of the client group taking into account physical, psychological, environmental, social behavioral, spiritual and cultural needs.Taking into account carers needs. * Provide professional guidance and support for Technical Instructor III's/OT Assistants. * To assess and treat patients and develop, implement, evaluate and modify individualised treatment program ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required. * To carry a caseload of patients that present with acute or chronic conditions * To participate in appropriate meetings e.g. MDT's * Assess, plan, prioritise and provide treatment, rehabilitation, equipment, adaptations etc. as appropriate to the client group. Assess for and advise on patient's care needs and evaluate this. Key Requirements of the Person Specification: * Degree/diploma in Occupational Therapy * Health and Care Professions Council Registration * Experience in treating patients with a variety of conditions * Able to work flexibly and respond to service requirements * Use or access to a car or suitable means of transport. * Experience of working in or having student placements in a variety of Occupational Therapy departments or settings.
