Applications are invited for a 9 PA Consultant post in Palliative Medicine based at York Hospital with the opportunity to do cross site working across the care settings in York Teaching Hospital NHS Foundation Trust. This post is to replace the existing established consultant, who is due to retire at the end of 2020. The post holder will be responsible for providing expert strategic support, alongside the medical leadership and clinical review and management of patients at the Trust. . This is an exciting opportunity to build on the excellent work the existing Specialist Palliative Care Team at York Teaching Hospital NHS Foundation Trust has been doing. We provide support for the full range of clinical teams working across the trust. We are a dynamic and friendly team consisting of Consultant, Lead Nurse for Palliative and End of Life Care, clinical nurse specialists in Palliative care (CNS/ Macmillan), and a medical secretary. The consultant will offer support and advice to the multi-professional community palliative care team. You will work closely with colleagues in acute medicine, frailty, critical care, acute oncology and front door services, attend cancer MDTs (MUO has a requirement for consultant attendance), and work collaboratively with community colleagues to promote individualised patient centered care and advance care planning. The role will support ongoing projects, initiatives and service development such as Advance care planning, ceiling of care decisions, and the disadvantaged dying. The post holder will be actively involved in promoting the improvement of palliative services across the locality and region, with a focus on how enhanced supportive care priorities and the NICE end of life care guidance can be integrated into the Population Health framework. There is a team located in both acute hospitals and a Community Palliative care team in York and each currently provide a 7 day service. There will be opportunity to develop and contribute to service improvement and patient safety. There is access to a well-supported audit and research portfolio an opportunity to develop this further. As well as this, the successful candidate will also contribute to teaching both undergraduate HYMS and nursing students and an array of postgraduate healthcare professionals. The medical director of St Leonard's Hospice, which is based in York is based a few miles from the hospital, also provides additional in reach PAs to the Trust. The Trust has a strong relationship with the hospice and they offer a wide range of supportive day services for people suffering from life limiting illnesses and offers care and support to patient's families. The Trust also has a strong relationship with St Catherine's Hospice which is based in Scarborough. York Hospital is located just minutes away from the historic city center of York which is home to various restaurants, bars, shops and theatres perfect for all ages. There are many bus routes accessible throughout York and the train station is only a ten minute drive from the hospital with regular services to surrounding areas such as Leeds and the ability to travel to London in under 2 hours.
