Overview

Job Description

At Bridgwater Hospital we are a joint led team of Therapists & Nurses who all possess the same focus, getting our patients back into their normal place of residence whilst promoting their independence along the way. We encourage patients to manage their own needs ensuring they are confident for their safe and proactive discharge. Waverley Reablement Unit boasts state of the art facilities in our newly built Hospital which is participating in an exciting new project working in conjunction with Musgrove Park Hospital - our HomeFirst Pathway and model of care. Here we have a positive, pro-active attitude towards the HomeFirst Pathway, working hard to ensure the stay in our Reablement Unit is both short and supported. Hear what NHS England have to say about HomeFirst: 'Patients avoid 7,500 nights in hospital with 'tailored' home help scheme' - scheme offers patients who are healthy enough, and their families, tailored help to finish therapy at home, with personalised care reducing stays in hospital by up to ten days. As a Nurse on the Reablement Unit, you will be responsible for the assessment of Patient care needs, the development, implementation and evaluation of treatment plans. You will be working alongside other professionals within the multidisciplinary team; ensuring high-quality care and effective communication is maintained. You should demonstrate a compassionate and caring approach to your care delivery, working with patients who have undergone surgery or been medically unwell to support their rehabilitation. Whilst the NHS is changing to be community focused; we can offer skill development opportunities in many areas of practice. The successful candidate will also be working within a developing model of care. Care is delivered on a personalised basis jointly developed with the patient and/ or carers. Bridgwater lies along both sides of the River Parrett & boasts close links to its larger neighbouring town, Taunton. If promoting patient independence interests you - we need to hear from you.
