Job Description

Then this may be the job for you! We have an exciting opportunity for a registered RGN/Practitioner to join the Grantham Integrated Community Team, you would be working within the CQC rated 'OUTSTANDING' Lincolnshire Community Health Services (LCHS) NHS Trust. Being a Community RGN is a privileged position where you would be responsible for delivering high quality, patient centred nursing interventions for individuals in their own homes, residential settings and clinics within Grantham and the surrounding villages. The skillset of a community nurse is vast and non-exhaustive, requiring knowledge and expertise in a range of areas. Examples of the interventions provided are; wound management, continence care, palliative care, medication administration and long term condition management. We aim to support and empower patients, families and carers to collaboratively plan individualised care, promote independence, and ultimately enable patients to manage their own nursing care needs where possible. We strive to prevent unnecessary hospital admissions and work closely with neighbourhood and other multidisciplinary teams to provide the safest and most appropriate care. Training will be provided to newly qualified nurses or those new to the community, we can offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential. At LCHS we are committed to developing our staff and consequently offer a range of accredited courses to support career progression from band 3-7 and beyond if desired. The RGN role supports progression via a managerial or advanced clinical skills pathway and the trust regularly offer developmental roles to support with the career pathway of choice. Post - RGN - 22.5-37.5 Hours per week, flexible shift pattern to cover a 7 day a week service. Shift patterns are generally 7.5 hour days 08.00am -20.00pm 7 days per week with occasional long days at weekends. Flexible and set working hours may be negotiable, providing it meets with service needs and will be reviewed periodically in line with local policy.
