Job Description

We are looking for an Occupational Therapist to join our team The Wandsworth Maximising Independence is a dual pathway for patients within Wandsworth who require rehabilitation and/ or reablement. The service is part of the Wandsworth Community Health Service, and prides itself with its integrated working. We work alongside our nursing colleagues, GPs and specialist community services to provide excellent locality based care to patients within Wandsworth. We work closely with our partners in local acute care providers, social care and third sector to better meet complex patients' needs. The team comprises of a large multidisciplinary team; physiotherapists, occupational therapists, speech and language therapists, rehabilitation assistants and support workers. It provides a urgent and routine pathway to adults over 16 years of age, working with them to regain their independence following illness or injury including falls, preventing avoidable attendance or readmission to hospital, supporting them to manage long term conditions and promoting health and wellbeing. So what do you need to join our team? * Experience of working in the community with adults to provide rehabilitation following illness or injury or would like to move into community work. * To lead the team and develop clinical pathways within Wandsworth * Ability to demonstrate working autonomously in developing treatment plans, assessment, discharge planning for patients with complex care needs/ end of life care * Highly motivated, passionate and enthusiastic team player * A positive attitude and commitment to providing excellent holistic care and evidence based interventions. * Experience of supervising others For full requirements please do look at the person specification for the role.
