Job Description

Looking for a Social Worker to join a team in Rhymney 37 Hours per week. Contract position. A vacancy has arisen in a busy Community Mental Health Team. We need an experienced Mental Health Social Worker who is able to undertake duties in the North of the borough. You will need to have a good understanding of current legislation and policy. You will be required to work closely with carers, other professionals and team members or alone using your own initiative. You will take part in a duty rota dealing with crisis situations and you will facilitate safe discharge for service users from hospital into the community. You will be required to support and safeguard vulnerable service users through difficult periods in their lives, therefore effective communication and organisational skills are essential. - To ensure social work is provided within the policies and procedures of the Directorate. - To ensure that the quality of social work practice meets the National Occupational Standards and Codes of Practice for social work. - To ensure that safeguarding concerns are investigated or communicated to the relevant person for action to be taken. - Assess and manage risks in relation to individuals, families, carers groups and communities. - Contribute to the management of resources and services. - Work within multi-disciplinary and multi-organisational teams, networks and systems. - To ensure that interventions with service users and carers are managed in a confidential context. - To prepare for and work with individuals, families, carers groups and communities to assess their needs. - To bring concerns in relation to the quality of social work practice, assessments and care plans to the Team Lead. - To ensure breaches of contract / concerns about the quality of service provision are brought to the attention of Contracting and Quality Assurance for action. - To ensure that the service is delivered within the equality commitment of Caerphilly County Borough Council and is anti-discriminatory and addresses the language and communication preferences of service users and carers.
