Band 6 District Nurse/Case Manager -Leamington North Placed Based Team Base: Crown Way Clinic CV32 7SF Salary: Band 6 Hours: 37.5 hours per week over 5 days Contract: Permanent Come and join a Trust rated Outstanding by the CQC. South Warwickshire NHS Foundation have been rated as outstanding following our latest inspection by the CQC and we are recruiting new staff to come and help us improve even further. Leamington North team are looking to recruit an enthusiastic, dynamic and motivated Band 6 District Nurse/Case Manager to help lead our community nursing team based at Crown Way Clinic. The team strives to provide patient centered care, and actively encourage prevention and self-management, therefore the applicant will be required to work closely with the multi-disciplinary team, working collaboratively, and strengthening relationships with our GP's. The successful candidate will have a minimum of 4 years' experience as a band 5 within the community setting, must be organised and flexible and be able to plan and prioritise their own workload. The candidate will be able to demonstrate their leadership behaviours and skills in managing the complex care of long term conditions and end of life patients. You will be responsible for a team of clinicians and in addition will support the two Band 6 colleagues within the placed based team with caseload management .You will have the support of a Clinical Practitioner, Professional Lead and admin assistant. The applicant will hold the community nurse prescriber qualification and a Specialist Practitioner, Long Term Conditions degree or have commenced the Health Needs Assessment Module 359. Ideally you will have a leadership qualification or willing to work towards.
