We have a unique team of Reablement support workers who assist older people to regain their independence after being discharged from hospital back to their own homes. Helping them to rebuild life skills and confidence which they may have lost due to illness or a disability. About the Reablement Worker role * To help create a Reablement plan which holistically supports the client to achieve their goals * To work with the team and other health professionals such as Occupational Therapists and Physiotherapists and to draw on your expertise as and when the client needs extra support * To signpost the client to support that will enable them to maintain their independence * Full & Part time hours available, various shift patterns within our operating hours 7am - 10pm * Part time: 4 days on 4 days off * Full time: 5 days on 3 days off About you If you have had some experience within the care sector, we would love to hear from you.
