Early Intervention Community Team is a Prototype that is being tested in a Community Setting to help discharge Patients earlier from the acute setting, but also preventing hospital admissions. Support is needed to roll out the Early intervention Community Team, which means expanding this to the South of the Birmingham. We are looking to recruit Bank Level 5 Registered Nurses to clinically support the Multi Professional Team within Birmingham Community Healthcare NHS Foundation Trust via the Trust Bank, the organisation's temporary staffing department. You will need to be a Registered Nurse who is willing to develop a new way of working and be enthusiastic for the challenge to take part in delivering Community Services in a different way within a Locality. We will be working with BCC and UHB to deliver care and prevent duplication of visits to keep the patient at home and promote independence. The post holder must hold a registered nurse (Level 1 - Adult) qualification and a current NMC Registration. We will not accept applications from unqualified or Student Nurses for this position. You will be a part of a large Community Nursing Team consisting of District Nurses, Community Staff Nurses, Health Care Assistants and Community Matrons. These Community Nursing Teams deliver a wide range of care to patients registered with Birmingham Community Healthcare NHS Trust.
