Overview

Spencer Clarke Group are working closely alongside Waltham Forest Council to provide them a with a (CMHT) Community Mental Health Social Worker to join their team on a Contract Basis Full Time Contract – 6 month agency contract with a view to being extended on an ongoing basis. FULL TIME Rate - £28 to £34 per hour - Negotiable dependent upon experience- Please submit CV with rate you require Your Role; Undertaking assessments and support planning for social care under the Care Act 2014. Carrying out Care Act Assessments/Support Planning and other social care tasks to enable a person to leave hospital and return to the community. Prioritisation of safeguarding referrals when personal welfare is believed to be at risk. Investigating safeguarding concerns and the development and monitoring of safety plans when appropriate Re-assessments of care packages with a view to ensuring that service user outcomes are met Attending CHC Joint funded/ section 117 related reviews and responding as appropriate. Prioritisation of all requests for AMHP involvement in Mental Health Act Assessments and wider practice. Mental Capacity Act related work.
