Our client specialises in providing person-centred support to adults living with Learning Difficulties. Many of the people we support also have additional complex needs including mental health, behaviours that challenge, Autism or Physical Disabilities. Who are we looking for? We are on the lookout for someone who will support the Service or Registered Manager in the day to day management of the service, ensuring that all service user care needs are met and all policies and procedures are adhered to. In addition; to support service users who have learning disabilities, communication difficulties and behaviours which challenge, to maximise their potential to lead an ordinary life. Key Tasks: - Work as part of a team leading the team whilst on shift and support the Service or Registered Manager to deliver care and support. - Support the Service or Registered Manager to supervise and manage the team to deliver quality service outcomes for service users, including acting as a key worker, developing and managing care & support plans and working with service users on a day to day basis - Support the Service or Registered Manager in developing activities and services to meet identified needs that will support individual/community well-being and individual and cultural identity. - As key worker, continually re-assessing service user needs against the current care plans and liaise with appropriate agencies should changes in the care plans be required.
