Overview
Care Coordinator (Assistive Technology) Location: Glenrothes, Fife, with significant Travel across Fife Salary: £25,000 pro rata Contract: 6 months Fixed Term contract, with opportunity to extend Hours : Full and Part-time positions available, we consider flexible working requests Note: this role is for an experienced Care worker or someone looking to start their career in care - Full training will be provided Just Checking provides activity monitoring services that help people get the right care and support. Using small wireless sensors placed around the house, Just Checking creates an overview of daily activity. This information helps professionals offer support that accurately reflects individuals’ needs, promoting person-centred care and helping people stay at home for as long as possible. Care Coordinator (Assistive Technology) - The Role: To take full responsibility for the installation / deinstallation of assessment monitoring systems and to gather the outcomes required from each installation. Responsibilities and Duties: Installation and deinstallation of Just Checking assessment systems as prescribed by Fife Council into people’s homes Monitoring up to 50 installations to ensure their effective operation Get an understanding of the required outcomes for each installation, modifying practice to suit the individual needs and sensitivities of each user Review chart summaries and ensure that they provide the relevant information to support effective decision-making Document and report the outcome(s) of the assessment to relevant practitioners Drive awareness of the service by conducting training and awareness sessions, attending meetings and providing information services to individuals, carers, families, professionals Support professionals and others to interpret and translate data Contribute to a monthly service review with local authority commissioners and practitioners Liaise with relevant professionals and key stakeholders, ensuring effective service delivery and resolving any issues as they arise Interdependencies include - service managers, social workers, occupational therapists, assistive technology teams, care and support providers and support staff, family members, Just Checking account managers, customer support and other assessment technicians Care Coordinator (Assistive Technology) - Candidate Specification: Able to organise your own caseload and time, prioritising and managing multiple demands Naturally comfortable working in adult social care settings and empathetic to the needs of people receiving support, ideally having experience of delivering care and support to either older adults or adults with learning disabilities/autism Personable, effective communication skills appropriate to a wide range of audiences (including adults who may have communication difficulties and professionals in a formal setting) Strong presentation skills - able to deliver training, manage meetings and articulate concepts well Able to work independently, self-motivated Excellent problem solving / analytical skills Accurate and effective recording and reporting skills Basic IT competency - practical knowledge of Excel, Word and web-based office software Experience in a social care role would be an advantage Hold a full valid driving license, with daily access to a vehicle and willingness to drive across the Fife region Successful candidates need to pass Enhanced DBS and PVG checks.
