Overview

Are you a Mental Health Support Worker looking for a new role? We currently have a fantastic opportunity for a Band 3 Support Worker to join an NHS Community Recovery Team based in Wanstead Applicant must hold a full driving license and have access to a car. •Do you know someone who would be interested in this role? We offer a £250 referral bonus per successful candidate that is referred Temporary pay rate: £11ph ltd Hours and length of contract: •Full time: Monday to Friday 09:00 - 17:00 •The manager is looking for the position to be filled ASAP with an initial 3 month contract length. Daily Duties: You will support the discharge facilitators with patient flows between mental health inpatient beds and the community to identify required tasks to support timely discharge and escalate any potential delays. Duties will include: •Ensure effective systems are in place for discharges from the acute Inpatients beds to the community •Communicate effectively with the Ward & Community Multi-Disciplinary Teams (including Social Services) to support discharges •Attend weekly bed management meetings •Support community services in liaison with primary care, local authorities, housing and third sector organisations to ensure discharges are timely •Chase up assessments and reports which are required for relevant funding panels •Liaise with Brokerage and Commissioners and make appropriate referrals to provider services •Support patient visits to provider services / accommodation for assessment of suitability Person specification: The applicant must be DBS cleared and have previous experience in the field.
