Overview

We are looking for enthusiastic and highly motivated nurses to join our teams at Norman Power, Kenrick and Perry Trees; we provide integrated care to patients throughout Birmingham Community Healthcare NHS Foundation Trust so the ideal candidate must be highly mobile and are able to travel throughout the City’s Boundaries. Early Intervention Community Team is a brand-new integrated team working with people in their homes and Promoting independence. Early Intervention Community Team is an integrated service which aims to build relationships and break barriers between organisations, whilst working effectively for the person, avoiding duplication and reducing delays and empowering staff to be decision-makers, and building trust with the wider system. Health and social care professionals across Birmingham are working more closely than ever before to help older people and others with complex needs to recover faster and live healthier and more independent lives in their homes. The results speak for themselves. More people are back in their own homes living more independently. More people are avoiding hospital admission and more people are recovering from a crisis faster. Our ambition now is to make these services and benefits available to people across the city in 2020. The post holder will be expected to work multi professionally, support other team member’s roles and contribute to the future development of the service. We are looking for people who are innovative, forward thinking, flexible and dynamic Nurse who will join this Multidisciplinary team to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge, in line with the overall objectives of the service. The ideal candidates must be a good communicator, team player and have sound clinical knowledge being able to work independently and as part of a team. The post holder will be expected to work shifts as required over 7 days a week during the services working hours of 08.00 – 22.00.
