Spencer Clarke Group are working closely alongside Waltham Forest Council to provide them a with a Hospital Discharge Social Worker to join their team on a Contract Basis Full Time Contract – 6 month agency contract with a view to being extended on an ongoing basis. FULL TIME Rate - £28 to £36 per hour - Negotiable dependent upon experience- Please submit CV with rate you require Responsibilities as a Hospital Discharge Social Worker: To promote and maintain independence for vulnerable adults To protect adults at risk using procedures to safeguard adults from abuse Assist and participate in the management of the duty service and carry out screening of clients to determine criteria to be supported by the service Expected to also work in the D2A (discharge to assess) Able to identify and carry out MCA Be part of the MDT and BI meetings when required on the wards Essential Requirements: Social Work England registration Eligible to work in the UK Hold a full UK licence Post-qualified experience If you wish to be put forward for this role, please apply and contact Khalsa Singh Spencer Clarke group – WHY SPENCER CLARKE?
