Overview

We currently have a great opportunity for a Qualified Social Worker to joint the Adult Social Services Team in North London. The main purpose of the role is to work with adults who are physical frail, disabled and elderly that may have a dual diagnosis. To be able to work in cases of emergency, prioritise workload, have a flexible approach to work. Main Duties: To manage a caseload of complex cases/assessments Manage and monitor a workload of complex, specialist and generic cases and organise work activities taking into account the need to prioritise tasks and responsibilities, ensuring statutory responsibilities are undertaken Undertake interviews, Mental Capacity Act assessments and observations and gather information from informal networks and other agencies to analyse summarise and evaluate the information to provide a holistic assessment of needs, balancing risk and protective factors Document cases accurately and concisely ensuring this is proportionate and only includes salient information Contribute to multi agency, multi disciplinary conferences and meetings as appropriate To communicate closely and sensitively with family members of service users Undertake all work in accordance with the required local and national policies, procedures, best practice standards and current legislation Essential skills & experience needed: To have sound knowledge around the continuing health care process and procedure and the legislation that underpins this, including the Decision Support Tools, London Health Needs Tool To have knowledge and understanding of risks, Safeguarding, Care Act 2014, Deprivation of Liberty and Continuing Health Care Ability to understand and consider the needs / criteria for sheltered, residential and nursing care Ability to work with challenging cases, flexibility and to be able to prioritise Experience of organising and managing support packages Job requirements: An enhanced DBS that is of satisfactory level is essential Must have a minimum of 2 years experience working within Adult Services All applicants need to be social work qualified with current SWE registration This job was originally posted as www.totaljobs.com/job/89933225
