Overview

We're looking for a Field Care Supervisor / Senior Care Assistant / Senior Care Worker to take up a position as a Senior Care Practitioner for community care company in High Wycombe. This is a Monday - Friday 9am - 5pm role but you'll need to be flexible and able to participate in the on-call rota. You'll be assisting with the management and supervision of Care Workers and management of service delivery through meeting with clients. On offer is a salary of £20,000 - £21,500. The Senior Care Practitioner role As a Senior care Practitioner, you'll work with the manager to ensure that a safe and effective service is provide to clients, through the thorough assessment and regular review of their needs, and ensuring that the team of care and support workers are well supported in their role and receive relevant training, coaching and supervision. Assisting with inductions through mentoring and coaching Identification of training needs Spot checks on care and support workers Hold team meetings to discuss new clients, changing client needs, practise issues and to disseminate good care practise. Coordinate service delivery and equipment to CSW's Undertake referral / assessments with client Gather, record, communicate and regularly review all client information Work in partnership with external agencies to ensure clients needs are net Identify any issues promptly that require further investigation and report to a senior manager Participate in the on-call rota Deliver care where needed The Successful Candidate This role would suit an experienced care worker, senior carer / senior care assistant / field care supervisor who has experience of supporting service users in the community. You'll have the ability to conduct care needs assessments and risk assessments and draw up care plans. You'll be able to participate in an on-call rota, and be flexible to occasionally work evenings or weekends to meet the needs of the service You'll be qualified to NVQ level 2 in care, and ideally hold level 3.
