Social Worker - Discharge to Assess Team - Swindon - £36 per hour We are looking for an experienced social worker to be based within the discharge to assess unit at Fessey House. The successful candidate may be expected to work flexibly across the hospital, SWICC and Fessey House depending on the demand. The ideal candidate will have experience working within a Discharge to Assess facility or experience in hospital discharge. Benefits as a Discharge to Assess Social Worker: Excellent rate of pay Flexible working Responsibilities as a Discharge to Assess Social Worker: To promote and maintain independence for vulnerable adults To protect adults at risk using procedures to safeguard adults from abuse With the adult and carer, formulate and implement personal budgets and support plans Assist and participate in the management of the duty service and carry out screening of clients to determine criteria to be supported by the service To be expected to also work in the D2A (discharge to assess) To be able to identify and carry out MC To be part of the MDT and BI meetings when required on the wards Requirements: Social Work England Registration Eligible to work in the UK Full UK licence holder Why Liquid Personnel?
