This post is for a Experienced Social Worker, to be based within the Discharge to Assess Unit at Fessey House. The selected candidate may be expected to work flexibly across the Hospital, SWICC and Fessey house depending on the demand. The ideal candidate will have experience working within a Discharge to Assess facility or experience in hospital discharge.
