Overview
Clinical Lead Nurse Carebridge is a healthcare company specialising in complex homecare for vulnerable children and adults. We currently support a number of patients with complex needs within the community ranging from those with epilepsy requiring rescue medications through to individuals with tracheostomies and whom are ventilator dependent. We work with NHS Clinical Commissioning Groups, Social Care and privately funded clients. You will be part of a specialised team. Carebridge is a Clinically Led provider supported by a team of experienced coordinators, recruiters, a Training and Staff Development Lead and our Managing Director who has 31 years' experience in care. At Carebridge we are passionate about putting CAREFIRST and enabling people to live safely in their homes and wider communities. As a Clinical Lead Nurse, you will be accountable for a case load within the community environment. You will be responsible for supporting and developing nurses and unqualified staff through clinical practice. You will be supported by a team to assess, plan and implement care packages working in partnership with families and the MDT, to ensure safe delivery of care. As a Lead Nurse you will assist the training, set up and review health and social care packages ongoing to ensure a skilled workforce is in place to deliver safe quality care to all patients. You will be required to promote evidence-based practice, working collaboratively with registered nurses and wider Multi-Disciplinary Team. As a Clinical Lead Nurse you will assist the assessment, training, set up and review of health and social care packages, and ensuring a skilled workforce is in place to deliver safe quality care to all clients. Some other responsibilities will include: Devising care plans and risk assessments Attending MDT meetings Carry out staff training, appraisals and supervision's Maintaining accurate and contemporaneous records Escalate safeguarding issues and manage risk Work collaboratively with a team A full JD can be provided Criteria You must be a registered nurse adult/ paediatric An active and clear NMC pin A clean DBS
