Overview
Social Worker - Adults Transition - Dudley - £23.26 per hour We are looking for an experienced social worker to join a adults transitions service in Dudley. As a successful social worker you will be carrying out assessment, reviews consistent with Care Act legislation. You must ideally have previous experience in a 0-25 disabilities service and / or transitions social work role. Managing a caseload of clients with learning disabilities - 0-25 services. Work in partnership with health professionals. Provide written and verbal reports as required and monitor the implementation of care plans. Undertake S42 safeguarding investigations and work closely together with relevant SAM. Promote independence of service users. Adept knowledge of both children's and adult's legislation.
