Overview

Quickline Recruitment currently have a fantastic opportunity for Social Worker to join the Discharge Team in Wiltshire Council. To be successful for this role, you are required to have previous experience within Adults Services or within a hospital discharge team About the Role The hospital teams are based in three acute hospitals and provide advice, assessment and discharge planning for people. All of these teams work closely with health colleagues to promote an effective multi-disciplinary approach. We’re currently looking at innovative new methods such as 'discharge to assess’ which is following people from discharge to their own home or care home. We’re also exploring innovative front door of hospital approaches to ensure the correct discharge pathway is instigated as soon as possible. Excellent rate from £32.00 to £36.50 p/h Requirements of the Social Worker Educated to degree level i.e. Diploma or degree in social work, CQSW, CSS or equivalent.
