Social Care Assistant Challenging times need caring people We are looking for caring and experienced people to support our vulnerable Adult residents within South Gloucestershire. In these extraordinary times you really can make a difference by working alongside our Social Workers, Occupational Therapists and responders, working with adults who may require social care services, their family and carers and assessing their social care, functional and health needs and to plan, implement, review and monitor actions and outcomes. What does an average day look like? - Enable hospital discharges by carrying out assessment of needs, providing advice and guidance to service users and their carers. - Identify needs and review and monitor care plans and the quality of service provision, responding to changing needs and renegotiating service provision where appropriate, taking necessary action in accordance with Safeguarding procedures in order to protect vulnerable people. - Provide advice, support and guidance to service users and their carers and the delivery of designated areas of work by coordinating day to day operations and contributing to developing projects and processes. - Raise awareness of a range of care issues and their implications and carry out risk assessments where appropriate, in order to ensure that needs are assessed, and appropriate action is taken. - Develop and evaluate reablement programmes as part of care packages ensuring that programmes to support independence are implemented. - Make recommendations for significant budget spend ensuring that maximum value is delivered for the resources deployed. What we need from you: - Experience in a relevant area of statutory or voluntary sector work in the health or social care field. - A relevant qualification e.g. NVQ Level 3 in health or social care or equivalent. - IT skills to use client data system. - Understanding of the needs of service users and good interpersonal skills. - Knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment. - Ability to carry out risk assessments.
