Overview

Adult Hospital Discharge Team in Bristol require experienced Social Workers to join their team. Immediate start and on-going contract. Paying £30ph. This role is to support hospital discharges from acute hospital settings and non acute NHS community settings. Use of creative solutions to agree timely discharge from hospital. Care Act assessments completed in a timely way to expedite hospital discharges, knowledge of MCA. Hospital discharge experience desirable but not essential.
