Overview
Reed Health and Care are looking for a Qualified Social Worker who is interested in working in Nottinghamshire, Hospital Discharge Team. This is a fast pace of work as we have to work towards the estimated discharge dates, we work in a multi agency team of social workers, community care officers, nurses and Physiotherapist. Duties will include : FACT FIND assessments Assessments Reviews Different pathways of care for discharge planning. Applicants must have a qualification in Social Work to degree level or equivalent (e.g. BSc, BA, MA, CQSW, DipSW) and be registered with Social Work England (SWE).
