Supported Living Coordinator Dewsbury, West Yorkshire Permanent, Part Time (25 hpw, 3.30 pm to 9.00 pm shifts) Starting salary circa £19,000 pa, plus brilliant benefits £9.00 per hour if you’d prefer to work on our bank on a casual basis Empowering customers to live rewarding and happy lives. We are doing things a little differently at the moment, so to keep you, our customers and ourselves safe we’ll engage with you digitally where possible. We’re currently putting the finishing touches to our independent supported living CQC services in the beautiful old mill town of Kirklees. Our new Queen Street service supports adult customers who have a range of complex needs including Learning Disabilities, Personality Disorders and Mental Health issues. We’re offering person centred support, helping customers to live in their own home and fulfil their goals and aspirations so they can live their best lives. Our beautifully refurbished buildings have self-contained furnished apartments. Queen Street are two buildings with open living spaces and a newly decked outside area (ideal for those summer BBQ’s). We are also very close to Huddersfield town centre by public transport or by car. Your role You’ll help change our customer’s lives for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans which are tailored to each customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. You’ll be brilliant at coordinating our Supported Living Workers to deliver on the support plans that you have created and work collaboratively with external agencies, to assess and engage support. You’ll also find that no two days are the same because everything we do is “person centred” around our customers. So you might be helping with personal care (issuing medication or bathing) in the morning, supporting them to cook their lunch and a bit of shopping in the afternoon. We even help customers to go on holiday Some of our customers have more complex needs which may include more support with personal care. About our team We’re recruiting a brilliant new team, so you’re going to be part of a fabulous new Home Group family. You’ll be reporting to Tracey our Registered Manager who celebrated her 16^th anniversary with Home Group this year, so you’ll have tons of experienced support on hand. Tracey loves nothing more than walking her dog Dink, before relaxing on the sofa with a good crime novel. She also has a real adventurous side, so you might also find her snorkelling off the Turkish coast or quad biking in the mountains. We're exhausted just thinking about it Working hours Hours are 25 hpw, 3.30 pm to 9.00 pm on a rota basis. You may from time to time be required to do some occasional weekend shifts in emergencies. Bank You can also join us on a flexible basis by being part of our bank. You can work hours that fit around your current commitments or pick up some additional shifts to your main role to earn some extra cash. About you You’ll have a positive can do attitude and passionate about making a big impact on our customers’ lives and shape these brilliant new services. It’s important that you have worked in a supported housing environment before and have experience of helping customers to reach their full potential. You’ll have a great understanding of professional boundaries, safeguarding, risk assessment, case management and creating bespoke and personalised support plans. Don’t worry, we won’t drop you in the deep end, we’ll nurture your talent, help you understand how things work at Home Group and you’ll have a fantastic induction with ongoing learning and support You’ll hold a Level 2 certificate or equivalent in Health and Social Care, if you also hold Level 3 this would be great, but don’t worry if you don’t, as we’ll pay and support you to achieve this. To make sure we keep our customers safe, it's really important that you are technologically savvy. When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service.
