Overview
Discharge Coordinator The post holder will work a part of the Complex Discharge Support Team to facilitate a timely, effective, and safe discharge of patients from the acute setting to the community. The post holder will be expected to work with NHS patients and Private Care patients as required in order achieve best outcomes for patients, optimise hospital bed utilisation and reduce length of stay. Location : South Kensington / Sutton Temporary : Expected to be ongoing Hours per week : 37.5 hours per week Salary: £23 (Umbrella) or £18 per hour (PAYE) Description of role The Discharge Coordinator will assist the multi-professional team by giving support and advice and acting as a resource for all aspects of complex discharge planning. This will include NHS funded continuing healthcare, where their role will be to support the ward to complete the documentation that accurately reflects the patient’s needs on discharge. The post holder will also play an important role in the on-going education of all appropriate staff to ensure they have the necessary knowledge and skills necessary to plan and implement safe and timely discharges. Excellent communication and collaboration with colleagues working in hospitals, community, and social and health services is required in order to promote a seamless discharge/transfer for all patients.
