Social Worker, Transitions Team Job Reference: L73-A-20-0361 Swindon Borough Council Swindon Salary: £31,369 to £32,640 p.a. 37 hours per week Transitions and Learning Disability Teams, Adult Social Care Swindon Borough Council are expanding and developing the Transitions and Learning disability team in Adult Social Care. These teams have separate line management, however work very closely together to form one larger team. The main legal framework both teams work within is The Care Act. Transitions Team Within the Transitions team, we work closely with young people between the ages of 14 years and 25 years who may have a learning disability, physical disability or mental health needs and support them to prepare to enter in to adulthood and develop their independence as much as possible. This enables you, as a professional to get to know a young person and their family over a longer period of time. You will be responsible for completing the Care Act Assessment in preparation for the young person turning 18 years old. During this time you will be working in a multi-disciplinary framework with education, health, children services and supported employment services amongst others before transitioning to Adult Social Care. Social workers are responsible for managing a caseload that may include capacity assessments, completing DOLS applications and attending MDT meetings. The successful applicant will have experience of and a passion to work with young adults who may have a learning disability, physical disability or mental health needs and are committed to providing a person centred and strengths based approach to their work. Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
