Overview

iPeople are currently recruiting for a Adult Services Assistant Care Co-ordinator. A Locum role you will be working with vulnerable adults as well as their families, with the main aim of making a real difference to their lives. iPeople are looking for Social Workers who is up to dealing with multiple cases and include the following skills: - Manage a caseload of adults with care and support needs and carers with support needs in a hospital setting Meet the expectations of the Care Act 2014 and its complimentary regulations and statutory guidance Undertake Care Act Assessment Support the needs of the service user's carers Post will be based at Wollescote Hall as part of the Reviewing Team. Applicants will be required to work 5 days a week (Monday to Friday) for 37 hours. To be involved in Care Management process under the overall responsibility of a Social Worker. To work with vulnerable adults to assess their needs and contribute to the development of support plans that enable people to achieve the optimum level of independence, with a need led review.
