Overview

As a Physical Health Nurse you will lead and co-ordinate all aspects of physical health needs of our patients working effectively with the individual, MDT and external professionals such as GP’s, SALT and General Hospital. You will be responsible for monitoring vital observations via the NEWs 2 national guidelines and identify any actions that need to be addressed. You will also be required to ensure that all physical health concerns are documented within the individuals care records and communicate this with the relevant healthcare professional both internal and external to ensure that the service user’s physical health is optimised. To be successful in this role, you'll need: First level nurse with evidence of professional development, relevant skills and knowledge. Awareness of the wider Healthcare economy and political environment Relevant experience in a similar environment Knowledge of NEWs2 National Guidelines Understand the physical health care needs for individuals who are living with a mental health condition Have a good understanding of the input required from external professionals such as GP’s, Occupational therapists, and SALT Excellent knowledge and understanding of how to communicate with individuals living with a mental health condition Good understanding of Infections control requirements and how some physical health conditions may impact on this role. Have a good understanding of the health action plan framework. As the Physical Health Nurse you will: Undertake physical observations and ability to analyse results. Communicate/liaise with external and internal professionals and share information appropriately Use CAREnotes for relevant documentation of areas of need. Complete physical health related assessments Organise any outpatient appointments for all individual within our service. Undertake phlebotomy needs Read Blood results and provide the results to RC for further review Provide reports and updates to the senior management team when required Be flexible in order to meet the needs of the service, this includes outside of normal working hours and at weekends/bank holidays when requested Where you will be working Location: Sturt's Lane, Walton on the Hill, Surrey, KT20 7RQ You will be working at Sturt House, a recovery focused, rehabilitation unit providing specialist mental health treatment for males aged 18 and over.
