Along with their allocated D&A practitioner, the Recovery & Reintegration Worker will assess the clients, track them throughout their sentence, ensuring that there are comprehensive release plans in place, and meets them upon release to "hand-hold" them through the first 24 hours following release, assertively linking them into treatment and support resources, like AA/NA meetings. A holistic release plan will be drawn up with the offender (in conjunction with prison based teams) in order that all necessary referrals can be made prior to release, and care pathways established for each individual client. All necessary appointments are pre-booked. What we are looking for? * Develop links with local housing providers to minimise the number of clients released with no fixed abode. * Liaise with medical services, where appropriate, to ensure clients have access to any necessary medical care upon release * Provide advice and support to the Drug and Alcohol Practitioners in their area in order to ensure the continuity of care for each client who comes into contact with Forward service are met. * Liaise with medical services, where appropriate, to ensure clients have access to any necessary medical care upon release * Undertake assessments of clients in prison, to co-facilitate pre-release groups in order to develop comprehensive release plans for our clients. * Develop links with other relevant agencies that offer aftercare support across England ensuring clients have access to appropriate support post-release regardless of their area of release. * Provide, where appropriate, addiction education and harm reduction information to clients who require it to avoid risk of overdose on release. * Participate in all staff meetings, support sessions, case conferences and supervision sessions as required by the line manager.
