Job Description
An opportunity has arisen within Hartlepool Psychosis Team for a Senior Mental Health Nurse to join a busy community team, providing evidence based care and treatment within a recovery framework for adults with Mental Health difficulties. You will need to be enthusiastic and highly motivated with experience in assessments, care planning and delivery of care for patients in a community setting. You will be expected to work as an autonomous practitioner in line with current legislation, policies and procedures. You will work closely with other professionals in the multi-disciplinary team in order to provide assessment and care co-ordination responsibilities for a defined caseload. You will be expected to provide supervision and support to other Team members and you need to have a leadership skills. As part of your job role you will be expected to mentor students. Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.
