Job Description

We have an exciting opportunity for a dynamic Rehabilitation Support Worker to join our Admission Avoidance and Discharge Service (AADS) in East London NHS Foundation Trust. This role is based in Mile End Hospital within the borough of Tower Hamlets. AADS is a team of multidisciplinary professionals that work across the community and hospital to facilitate early discharge and prevent unnecessary hospital admission. The service uses the discharge to assess model and works closely with our social care partners, including the Reablement service and hospital social workers. We have recently secured additional funding to expand the service and will be advertising several different roles in addition to this one, including OTs and physiotherapists. We are looking for a highly motivated, innovative, and flexible rehabilitation support worker to join our team in working with patients across a variety of clinical areas. The successful candidate will work under the direct supervision of registered health care professionals in managing complex cases and facilitating rehabilitative programmes in the patients home environment. You will work towards patient centred goals as set out in their support and treatment plan. The team has an excellent working relationship with hospital staff, GPs, the extended primary care teams, ambulance crews, the Physician Response Unit, care home staff and other community services. The work is varied and challenging and your role is to support the multidisciplinary team across the service when required. The team uses electronic patient records, so general IT knowledge is essential. AADS operates a seven day service, covering core hours between 8am and 8pm. The post does not require weekend work, however you will need to be a versatile and adaptable team player as this may change in the future depending on operational needs.
