An exciting opportunity has arisen for a Learning Disability or RMN with both mental health and learning disability experience to join our mental health in-reach team at HMP Lindholme. This brings a great opportunity to embrace change and embed culture. We have a nursing team that provides a stepped care approach offering In-reach and Forensic models of engagement similar to those experienced and replicated within the wider community. What will you be doing? As the complex care nurse within our team you will be involved in: Co-ordination, screening, assessment, planning, implementation and evaluation of individual Patient programs of care. Managing a caseload of patients deemed to have mild cognitive impairments, a diagnosis of ASD, ADHD with a focus on risk assessment and risk management strategies, ensuring that the requirements of the Care Program Approach are met in a timely manner. Multi-disciplinary care planning, both within the clinical setting and also in partnership with prison. Supporting group interventions Providing clinical supervision for junior members of the team Raising awareness about the mental and physical health needs of adults who have a learning disability Identifying and delivering awareness training needs regarding learning disability needs for healthcare and prison staff. What we need from you? The post-holder will be expected to work autonomously using their own initiative and as part of a multi-disciplinary team working in partnership with other services, agencies and organisations. They will also require OR be interested in developing specialist clinical skills in working with an offender population with a diversity of presenting problems, including psychosis and personality disorder, that feature alongside their learning disability and/or autism spectrum disorder. Good working knowledge of the Mental Health and Mental Capacity acts and health and social care pathways are essential as well as experience working with complex cases in the field of Learning Disability and/or Autism. Further Information We provide regular support and supervision by the clinical team manager, with the opportunity for peer supervision and networking with Learning Disability nurses across prison sites. Care UK also provides and supports ongoing professional training and development. The team operates a 7-day service between 08:00-16:00. 37.5 hours per week. There will be an expectation to work occasional weekends between the hours of 08:00-18:00. In addition, there may be an expectation to work longer shifts as necessary based upon rota planning and service development needs. The salary for this role is up to 37,500 per annum depending on experience. Employment Type: FULL_TIME
