Job Description
* Salary: £26,970-£33,779, dependent on experience * 40 hours, permanent contract * Equivalent to NHS Band 5-6. * Laptop / mobile phone provided * Mileage: 40p per mile We're looking for a leader for our Mental Health Recovery Team where you'll have the freedom to deliver effective rehabilitation and recovery for individuals presenting with severe and enduring mental health needs and associated clinical risks around their diagnosis. Specialist Care Team is a local, family-run, social care provider based in Morecambe and we work across north Lancashire in homecare, supported living and residential services. We work with Lancashire County Council and various CCGs around Lancashire to provide a Mental Health Rehabilitation step-down service with a goal to help individuals transition from a hospital setting to independent living in the community. You will be working directly with six individuals on the programme who all live in their own flat, which is set to grow to 12 people over the coming year. We are a dynamic organisation that strives for innovative change and improvement, so you will be in a position to help shape the direction of the service. Our two-year programme has a focus on promoting rehabilitation and recovery through specialist support to help the individual make choices, take control and progress to independent living. You will be leading a team of support workers in our high-quality recovery and support service in a therapeutic environment where you will be ensuring the service is meeting the needs of the individual and working directly with them on their recovery. The types of people we work with are: * Those with severe and complex mental health needs who have become 'stuck' and non-progressive in their recovery. * People whose needs cannot be met by less intensive mental health services. * When there has been an erosion of therapeutic optimism within mainstream services towards a person with complex needs, which may be hindering their recovery. * When a person is facing a transition from a highly supported setting to a less supported placement; this includes people leaving forensic or secure services, people leaving out-of-area placements, or leaving residential care to live in the community. * When a person needs help in overcoming disabilities associated with severe and complex mental health needs that would benefit from a structured environment and intensive therapeutic programmes. How will you be doing this? * Enable the Service User to meet all identified needs within the Service User's individualised Rehabilitation Care and Support Plan * Deliver evidence and outcome-based care * Assist Service Users with moving on to other more suitable accommodation where the need for support has reduced or no longer exists * Use a outcome-focused goal framework * Continuously review the achievement of, and progress towards, outcomes, enabling Service Users to gain greater independence and contribute to informing regular reviews In your role, you must be able to: * Recognise the specific mental health needs, including those associated with dual diagnosis and to develop approaches to respond to these * Monitor Rehabilitation Care and Support Plans and report progress against the plan, by using a goal framework for a strengths-based approach to supporting positive change around service user led outcomes and priorities * Developing approaches to enable individuals to manage daily living activities, such as self-administration of medication; accessing training/employment opportunities and other key daily living skills * Develop move-on plans for individuals who require ongoing support when rehabilitation has been completed * Have an understanding of the chronology of the disorders, but maintain a holistic focus in addressing the substance misuse, psychological, social and physical health problems * Use problem solving techniques to manage issues and build coping strategies to manage known risks * Provide a flexible, person-centred, empathetic, non-confrontational and non- judgemental approach which is important for maintaining an appropriate intervention programme * Work as part of a multidisciplinary team in the following areas: discharge, throughout the recovery process (including developing and responding to relapse prevention plans) and building up to a move-on plan. * Ensure support workers recognise and understand the individual's mental health needs; their recovery/relapse plans and the importance of risk assessment and the concepts of hazard, risk and control measures * Providing therapeutic support on a sessional basis, using assessment and intervention frameworks, influenced by evidence-based research You must hold the relevant qualifications and be HCPC registered to be considered for this role.
