Job Description
Macmillan GP Clinical Lead Cancer, Long Term Conditions, Palliative and End of Life Care Part Time (12 hours per week) Salary £101,383 per annum (pro rata) This post offers an exciting opportunity to take forward current best model practice within the key role of Primary Care Clinical Lead for Macmillan Cancer and End of Life Care and integrate this with work focussed on managing individuals with complex needs. This will focus on providing valuable primary care leadership within NHS Forth Valley in the implementation of Scottish Government priorities regarding cancer and complex care and end of life care in primary care. The aim of this essential leadership role is focus on quality improvement and to improve the care, outcomes and experience of those living in Forth Valley who have cancer or who are living with a progressive long term medical condition where end of life care is recognised as a potential outcome. There is also an emphasis upon developing the provision of excellent anticipatory care and end of life care services across NHS Forth Valley. The post holder will also be supported to represent NHS Forth Valley at appropriate Regional and National levels when required and to work with colleagues around Scotland to focus on improving the quality of care of people with cancer or complex healthcare needs. Work will involve close working as part of the Primary Care Leadership and Cancer and Specialist Teams in collaboration with Associate Medical Directors and Clinical Leads in Primary and Secondary Care. Aspects of the remit will be shared with the Nurse Consultant for palliative care, the Acute Hospital Palliative Care Lead and the Clinical Lead for Strathcarron Hospice. The post holder will work with other Clinical Leads to develop best model care pathways for individuals with long term conditions and resulting complex care needs and be enabled to develop close working relationships with Forth Valley key stakeholders in order to promote excellent care for those with cancer, long term conditions, multiple morbidities and life-limiting palliative conditions.
