Job Description
An exciting opportunity has arisen at Northamptonshire Healthcare NHS Foundation Trust. We are looking for inspiring Nurses with excellent leadership skills to join our team of dedicated community mental health team in covering Wellingborough & Eats Northants. The service aims to provide an excellent standard of person centered planned and unplanned nursing intervention to patients in their homes and is part of an integrated service that supports patients to remain in their home environment. As a County wide Community Nurse you will work closely with the multi-disciplinary team in order to support the service and have a high focus on delivering quality outcomes and improving the patient experience. The successful candidate will deliver safe, effective care for the patient experience for our patients and their families, who are predominantly housebound and have complex needs. As such you will need to be enthusiastic, innovative and highly motivated. Your personal qualities will mean that you have the ability to deliver a high standard of care in a supportive and forward thinking environment. It is intended that the service will move to postcode working, meaning that you will be expected to see patients within a given geographic area and may mean that you will need to liaise with a variety of GP surgeries.
