About you: An enthusiastic and experienced, registered Mental Health Practitioner, (Nurse, Social Worker or Occupational Therapist). You will have specialist knowledge of the care of Older people with mental health conditions. You will work in our Community Mental Health team providing mental health care and treatment using appropriate pathways including the Dementia Pathway in line with NICE guidelines. A good understanding of Mental Health Framework and legislation; the Mental Health Act & the Mental Capacity Act as well as Clinical Risk policies and procedures is desirable. You will have excellent communication and people skills, a warm personality and the resilience to help us transform and make a positive difference to our patients' lives. Caring for patients in the community, you will work in the Ascot, Windsor and Maidenhead area and must have access to your own car. What's next?
