Support Worker Young People Hounslow £23,200 per annum 40 hours a week Our client is currently seeking Support Workers for a house which is a 27 bed mixed gender service catering to Young People between the ages of 16 to 25 with medium to high support needs. The support worker will be allocated a caseload of 6-8 to key work and support, creating person centred risk assessments , support plans and creatively engaging Young Parents in order to minimise risks and increase their independence living skills. YP support planning needs to address issues around Mental health, substance misuse and engagement with Education, training and employment. The support worker will be required to work with the team to fulfil all day to day administrative tasks such as health and safety, income collection, reporting and follow up of repairs. Partnership working is essential to the role, support workers are required to build rapport and liaise with all external agencies working with young people. Support workers will be required to compile monthly reports for social services and attend meetings with YP and their allocated social worker as well as personal advisers where required. This is a 24 hr service and all candidates will be expected to work 7 days a week on a mixture of shifts of Early shifts 07:45 to 15:45 and Late shifts 14:00 to 10:00.
