Homecare Worker

Homecare Worker Nottingham Adult Services £13 per hour 3 Month Ongoing Candidates will provide homecare to citizens who are either awaiting discharge from hospital or have been referred from the community. The workers provide a range of essential support relating to medication management, reablement to support a return to independence, assistance with personal care and mobility and other activities to support independent living. They will work specifically within the 'Reablement' team, receiving referrals for new homecare packages. Part of this team's remit is to support safe discharge from hospital and also reduce hospital admissions through intervention prior to crisis. Access to own vehicle is essential, as care is delivered in citizen's own home. Candidates are required for AM shifts (7-1 or 8-2) and PM shifts (5-11) Shifts are worked according to a rota, with five shifts in Week 1 and two shifts in Week 2.
